Office Hours:
- Southern Blvd: Monday, Wednesday, and Friday 8 a.m. – 5 p.m. Tuesday and Thursday 8 a.m. – 6 p.m.
- Westchester Ave: Monday – Thursday 8 a.m. – 6 p.m. Friday 8 a.m. – 5 p.m.
By early February, convenient online scheduling and patient portal access will also be available.

Will there be any increased cost for care at AdvantageCare Bronx?
No. We accept the same health plans. Co-pays, deductibles, and other cost-sharing will not change.
Additional in-network care is available through the broader AdvantageCare Physicians practice and its care collaborators in the areas of:
Optometry • Rheumatology • Endocrinology • Nutrition • Behavioral Health • Dermatology (Advanced Dermatology) • Hematology and Oncology (New York Cancer & Blood Specialists).
In-network referrals for Pain Management and Neurology are also available.

At AdvantageCare Bronx, your privacy is our priority. We will continue to maintain your medical records. If you need a copy of medical records, simply follow the instructions below.
To make a request by mail, please access our Authorization Form, giving our staff appropriate permission to release your records. Please print and complete the form, with the patient’s or guardian’s signature, and provide a copy of a legal identification (passport or driver’s license).
